Families Clinics Online Booking System for Civil Service Eligible Persons
Introduction
Families Clinics Online Booking System for families clinics of the Department of Health (the System) will be launched in 2025 in phases. This new online booking system offers same-day appointment and appointment for the next calendar day (except Saturdays, Sundays and Public Holidays). Civil Service Eligible Persons (CSEPs) Note 1 who have their activated accounts can log in to the System to make, check, change and cancel their appointments Note 2 at any time via mobile devices or computers.
From 30 December 2024 (8:30am) onwards, CSEPs can submit their e-Registration Forms, namely “Application for Families Clinics Online Booking System Account”, on GovHK via the weblink on the websites of Civil Service Bureau or Department of Health or on the @DH Mobile App . Applicants have to use iAM Smart+ to electronically fill in and digitally sign the e-Registration Form for submission. Successful applicants have to activate their accounts in the Families Clinics Online Booking System within eight months for using the new system.
How can I get and activate my Families Clinics Online Booking System account ?
Submission of e-Registration Form (the e-Form)
- Make sure your account is “iAM Smart+”
- Apply for an “iAM Smart+” account / Upgrade your account to “iAM Smart+” in advance (if needed)
- Read the “Notes to Applicant” and “Collection of Personal Information – Statement of Purposes”
- Prepare the required information of applicant(s)
- Go to e-Registration Form “Application for Families Clinics Online Booking System Account”
- Suggest filling in the e-Form with the “e-ME” form filling function
- Enter the personal information of other applicant(s) attached to the major applicant’s e-Form. One e-Form can accommodate a maximum of six CSEPs in the same family (including the applicant).
- Ensure the email accounts provided in your e-Form are correct which will receive correspondences relating to the application
- Receive an email of “Acknowledgment of Form Submission” from GovHK upon successful submission of the e-Form
- Receive an email of application result (successful / unsuccessful) from the Department of Health within 14 working days

Note 2 The booking function of the Families Clinics Online Booking System will be implemented in phases. Changing and cancelling appointments are only applicable to the appointments made through the Families Clinics Online Booking System.
