Case Reporting of Heavy Metal Poisoning

We would like to seek your attention and cooperation in reporting patients with heavy metal poisoning to the Department of Health. Besides affecting individual patients, heavy metal poisoning may have a public health dimension to control the source. Hence, case reporting is important to allow prompt investigation and control of heavy metal poisoning to protect public health. For reporting purpose, doctors can fax the attached reporting form or contact the respective Regional Office of Department of Health at the following numbers:
