(The application can be submitted via fax, email, in-person, or through the government online e-form. For air-to-air transhipment applications, please download the PDF file and submit it via fax. In all other cases, submission using either the PDF form or the government online e-form is accepted. Regardless of the submission method chosen for import applications, the verification of a physical copy of the identity documents and relevant death documents is required.)

Application for Cremation Permit

Remarks: Applicants can either fill out the PDF application form and submit it with the relevant documents in person at the Cremation Permit Office, or fill out the online form to pre-submit the information for pre-approval. Regardless of the method chosen, whether applying in person or online, both require a single visit to the Cremation Permit Office to present the original documents for verification before a Cremation Permit can be issued.

Medical Certificate (Cremation) [Form 2]

Remarks: This form (Form 2 is one of the required documents for applying for a cremation permit) is to be completed by the medical practitioner. If the deceased has implants that have not been removed, the medical practitioner should provide additional information on Form 2. For a cardiac pacemaker that has not been removed, specify the model and the manufacturer. For other implants, provide the material and location of the implant. For radioactive implants, include the type of radioactive substance used, the radioactivity, and the reference date for the stated radioactivity.
